Intraperitoneal Hyperthermo-Chemo-Perfusion(I.H.C.P) - Clinical Analysis of Gastric Cancer Patients with Peritoneal Metastasis -

Abstract
30 far-advanced gastric cancer patients were received IHCP after surgical resection since 1990. There were 17 P0 and 13 Pl-3 patients. 27 patients were confirmed to have serosal invasion; T2(3), T3(21) and T4(6). Lymph node metastases were
positive
in
28 patents; NO (2), N1(6), N2(17) and N3(5). Lmmediately after radical operation, closed-circuit IHCP using 1.5% peritoneal dialysis solution with 10§¶/ml of Mitomycin-C warmed at inflow temperature of 44.5'C to 45.7'C was performed for 120 min
under
hypothermic general anesthesia. 3 year survival rate (YSR) was 88.2% in P0 and 2 YSR was 55.6% (5/9) and 50% (1/2)in P1and P2 respectively. However all 2 P3 patients were dead within 1 year. 3 out of 17 P0 patients were recurred; 2 at anastomosed
site
and 1 at peritoneum. 4 out of 9 P1 patients died; 3 of intraabdominal tumor recurrence and 1 of hematogenous metastases. 3 out of 4 P2 & P3 died of progressive peritoneal metastases but 1 P2 patient who had complete cytoreductive surgery is still
good
for 2 years. In conclusion, P1 and P2 can be treated by complete cytoreductive surgery with IHCP but there's some limitation in deep-seated cancer cells(P3, P4 & N3).
